Number of inpatient days and discharges
Staff trained to conduct Root Cause Analysis on each fall
Interventions identified and implemented
Incident entered into incident reporting software
Success measured by noting a decrease in repeat falls over time.
8 of 11 experienced decrease in numbers of falls.
