Project RED: Module 1: Preparing to Redesign Your Discharge Program

Provides a four-module training program to help hospitals implement Project RED.

Discharge Advocate

  • Is notified when patients in target population are admitted/diagnosed.
  • Initiates action steps associated with Project RED.
  • Initiates Patient Care Plan.
  • Educates patient and family about condition, medications, other treatments, post-discharge plans, and follow up ordered by the physician.
  • Reviews plan with patient and family.
  • Collects measurement data specific to project and patient population.

Notes:

In thinking about who may be identified to be your DA, it's important to note the patient population that has been identified for the project and the number of discharges per day to ensure that adequate resources have been identified to support the work. When piloting Project RED, consider identifying someone to serve as DA in a part-time capacity. Some hospitals have assigned a case manager to be the DA for half of every day; others have used nurse clinicians or clinical nurse specialists who are already working closely with patients in the targeted patient population (e.g., heart failure patients). Some hospitals are utilizing a nurse who may be on light duty for a few months; still others have split the role between two people, alternating days the person serves as DA. Flexibility is key while piloting Project RED to get a feel for what it takes by characteristics and by time to fill the DA role.
