Project RED: Module 1: Preparing to Redesign Your Discharge Program
Provides a four-module training program to help hospitals implement Project RED.
Primary Care Physician Referral Base
- Leaders will identify the PCP referral base.
- PCP satisfaction will be assessed prior to project launch.
- Physician champion will communicate with PCPs about project.
- PCPs will advise how to handle their off-shift and weekend patient needs.
The emphasis here is that the leaders need to help in identifying the hospital's PCP referral base, and the referral base needs to be accepted. This can be problematic: Where do you find a PCP for a patient who doesn't have one?
What will be your back-up plan for patients within your project population?
