Project RED: Module 2: The Re-Designed Discharge Process: Patient Admission and Care and Treatment Education: Slide 27

Provides a four-module training program to help hospitals implement Project RED.

Nursing Staff

  • Provide nursing care as planned
  • Educate patient and family
  • Communicate with each other
  • Communicate with other members of the health care team, including DA
  • Participate in multidisciplinary rounds, including those that may be specifically focused on discharge planning

Notes:

The DA, often a nurse, will need to be especially mindful about the role that the various members of the nursing staff play. Nurses usually educate the patient about his or her condition, diet, and medications. This education does not need to stop. In fact, having the DA repeat the education is probably beneficial to the patient. Further, the DA will likely have fewer interruptions than the assigned nurse who is also caring for other patients.

Consider the various nursing staff positions and how each works with the patient: Nurse manager, clinical specialist, assigned nurses, and patient educator. Your team will need to understand what roles these staff members currently fill in the discharge process and deliberately outline any changes in collaboration with these staff members.
