Case managers are very involved in discharge planning. Clarifying how the DA works with the case manager is an important step in re-engineering your discharge process. Case managers are experienced professionals, usually nurses or social workers, who develop close relationships with patients and may provide education related to the patient's condition and arrange post-discharge services. In addition to the education that case managers provide, the DA will provide daily education and the final teaching at discharge. Close communication between the case manager and DA should include discussion of the educational content each is providing as well as discussing post-discharge needs and services.
