Project RED: Module 3: The Re-Designed Discharge Process: Patient Admission and Care and Treatment Education: Slide 12

Provides a four-module training program to help hospitals implement Project RED.

Post-Discharge Services

  • Confirm with case manager that all services have been arranged
  • List services and contact information in Patient Care Plan

Notes:

The DA should confirm final arrangements have been made for post-discharge services that the patient will need at home. If discharge planning has been occurring throughout the patient's stay, the DA should check that all services noted on the discharge order have been arranged. Providing the names of companies providing services will help the patient and family contact them if necessary.
