a Data include Medicare beneficiaries discharged from January 2003 to December 2003 for 2003 data, from January 2002 to December 2002 for 2002 data. Readmissions are supposed to be counted in the numerators according to the measure specifications, but were not counted because the data were not available at this time.

b Per measure specifications revised 6/22/2005, percents and standard errors are calculated based on data weighted to reflect the contribution of each State's sample to national estimates.
