a. Consistent with the Agency for Healthcare Research and Quality (AHRQ) Patient Safety Indicators (PSI) software, admissions are identified using all diagnoses, including those present on admission. Exclusions include admissions with a diagnosis of cancer or in an immunocompromised state. Rates prior to 2008 are not reported because of International Classification of Diseases, Ninth Revision coding changes.
b. Rates are adjusted by age and gender using the total U.S. resident population for 2000 as the U.S. standard population; when reporting is by age, the adjustment is by gender only; when reporting is by gender, the adjustment is by age only.
