Text Description is below the image.

This is a bar graph of AHRQ Patient Safety Indicators for a sample hospital in West Virginia. It measures the State range (from 10th-90th percentile), State median, and the Hospital Risk Adjusted Value for a number of different conditions. The indicator rate is measured per thousand cases.

Below is a tabular description of the data.

PSI report for a single hospital. Note, Prday indicates the PSI was scored without taking into account the number of days from the procedure. We did not have procedure day in the data. Hence, the PSIs that use prday are no prday. The PSIs without a median bar had a median of zero and fell off the log graph.
