Text Description is below the image.

The graph shows rising numbers of patients in CC PCPs receiving the test. The fraction of patients rises from 0 to 0.7. The graph shows only CC PCPs with 10+ patients. The overall level = 30%. The PCPs at the lower and higher ends of the graph have statistical significant difference greater than .05 from the overall level. Each CC PCP is represented by a circle. The symbol size on the graph is proportional to the number of patients, range 10 to 335. The curve on the graph is lower than the curve on the previous slides, rises slowly and rises sharply toward the end.
