Project RED: Module 1: Preparing to Redesign Your Discharge Program

Provides a four-module training program to help hospitals implement Project RED.

Define the Current State

  • Initiate a high-level process map.
  • Multidisciplinary participation.
  • Patient admission is the starting point.
  • After hospital care provision is the ending point.
  • Ask each discipline what steps it takes to prepare the patient for discharge.


Let's talk now about how you know that your discharge system is working or is not working and the variables for success or failure. Before you move into establishing the new process for discharge included in Project RED, it will be important for all members of the team to have a working knowledge of what each member in the discharge process does. We often assume that we know what each other does when we discharge a patient, but surprises often occur when teams design a high-level process map. As part of your homework for this module, you will design a high-level process map of your discharge process. Please include all of the multidisciplinary staff members who are involved in your scoped project work area for the patients' discharge. Your process will begin with patient admission and end when your communication with the patient is complete. That may be on the day of discharge, or it may be during your post-discharge phone call if you have that process in place. The important part of this task is to define the current process so that you can identify what is and is not working and can move into the redesign phase later on in your training. There are no right or wrong answers in your high-level process map. The more honest you are about current state processes, the better able you will be at establishing root cause variables that create broken systems.
