"The DA will need to be especially mindful of the role that members of the nursing staff play. Nurses usually educate the patient about his or her condition, diet, and medications. This education does not need to stop; in fact, having the DA repeat information is probably beneficial. Further, the DA will have fewer interruptions than the assigned nurse who is also caring for other patients.
The nurse manager, clinical specialist, staff nurses providing daily care, and patient educator may all interact with the DA to help plan a comprehensive, patient-centered discharge program. The important point is to understand what roles nursing staff currently play in the discharge process and then to collaborate with them on any changes to the process."
