Graph showing the rate of adverse events due to medical care per 1,000 discharges for the years 1994, 1997, and 2000. Three values were calculated for each year:

  1. Accidental laceration or puncture during procedure per 1,000 discharges (excluding obstetric and neonate admissions).
  2. Iatrogenic pneumothorax per 1,000 discharges (excluding patients with trauma thoracic surgery, lung or pleural biopsy, cardiac surgery and neonates).
  3. Postoperative abdominal wound dehiscence per 1,000 abdominalpelvic-surgery discharges (excluding obstetric and neonatal conditions).
